
AFTERCARE PROGRAM

WHAT IS AFTERCARE?
The Aftercare Program immediately follows the completion of the 12-Step Residential Treatment Program. The program is a structured yet flexible continuation of care designed to support long-term success and prevent regression as the individual transitions from a highly structured environment to a more independent style setting. The program is person-centered, with services tailored to meet the individual's unique developmental, emotional, behavioral, medical, vocational, and social needs.

The primary goal of the Aftercare Program is to maintain and build on the progress made during residential treatment by promoting:

Ongoing skill development (life, social, vocational)

Emotional and behavioral regulation

Independence within a safe and supportive framework

Community integration

Long-term mental health and well-being

This program combines on-site supports, community-based activities, and transitional living options. It may last 6 months or more, depending on the individual's need. Core components of the Aftercare Program​
works together holistically to create a continuum of care at a more advanced level that supports the young adult’s journey toward partial or full independence.
